NCM Photo Agreement
All photographers must fill out a Photo Copyright License Agreement. It is the photographer’s responsibility to secure permission from all parties in a photo before sending it to NCM. It is not necessary to sign this agreement. Don’t forget to fill in the contact information!
All photos must have a Photo Documentation Sheet. This will ensure that the caption that accompanies the image is accurate. If you would like your images to just be considered for the large photographic mosaic mural, you do not need to send in a Photo Documentation Sheet.

Instructions for sending images to Synergy Design Group
- We are only accepting digital files. This means pictures taken on a digital camera or pictures that have been scanned.
- Please label each file as “Your last name-01,” “Your last name-02,” etc. Fill out a photo documentation sheet for each file, making sure to put the file name on each sheet. This is the only way we can match the sheet to the file.
- Submit one Photo Copyright License Agreement for all of your pictures. Fill out one Photo Documentation Sheet for each photo. You can email these forms to Synergy Design Group or mail them.
- You have two options for submitting your images.
- Email files (.raw, .tiff, .jpg, or .gif) to Synergy Design Group at:

- File requirements:
- Files should be at least 300 dpi or the highest resolution possible.
- Please take pictures on the “finest setting” your camera will allow or scan pictures at the highest resolution.
- The bigger the file, the more likely it will be used for the exhibits.
